Welcome to Amber's Home Blood Collections. By using my mobile phlebotomy services, you agree to the following terms and conditions:

 

1. Services Provided

 

Amber's Home Blood Collections provides professional mobile phlebotomy services, including blood collection and safe packaging of samples for posting to accredited laboratories at clients request. Samples are sent directly to the laboratory, and all test results are returned to your referring doctor or healthcare provider. I do not provide medical advice, diagnosis, or interpretation of test results.

 

2. Client Responsibilities

 

  • Provide accurate and complete personal and medical information prior to the procedure.
  • Ensure a clean, safe, and well-lit environment is available for the blood collection.
  • Be available at the agreed appointment time. Late arrivals may require rescheduling.
  • Follow up with your healthcare provider for test results and medical advice.

 

3. Consent and Acknowledgement

 

By booking and receiving phlebotomy services, you consent to the procedure and acknowledge that:

 

  • Minor side effects such as bruising, swelling, or fainting may occur.
  • The phlebotomist is not responsible for interpreting results or providing medical advice.
  • You assume responsibility for any adverse reactions, except those caused by gross negligence.

 

4. Appointment Cancellations and No-Shows

 

  • Cancellations or rescheduling must be made at least 24 hours before the appointment.
  • Late cancellations or missed appointments may incur a cancellation fee as outlined at booking.

 

5. Confidentiality and Data Protection

 

Your personal information is collected and handled in compliance with data protection laws and our privacy policy. We will only use your data for purposes related to delivering our service and will not share it with third parties without your consent unless legally required.

 

6. Payment Terms

 

  • Payment is required at the time of service unless otherwise agreed.
  • Accepted payment methods include cash or bank transfer.
  • Outstanding payments may incur additional charges.

 

7. Liability

 

Amber's Home Blood Collections and its representatives shall not be liable for any indirect, incidental, or consequential damages arising from the services provided. Liability is limited to the amount paid for the service. This does not affect your statutory rights.
